Tattoo Appointment Request Form

Please completely fill out the form below. All fields are required except preferred artist and reference images.

You can include up to three reference images in jpg, jpeg, or png format only. Each file has a maximum file size limit of 1MB.

Once we have reviewed your request we will contact you to schedule a time and day. We will also send an invoice so we can take a deposit. Your appointment can not be held until the deposit is paid.

Only the person receiving the service is allowed in the procedure area. Any guests will be asked to wait in the lobby. We appreciate your understanding as we continue tattooing our clients in a safe and sterile environment.
